Guidelines for Disability Forms

•  $25. fee for all forms

•  72 hour waiting period for disability papers from patients

•  Patient must complete their portion of the disability forms.

    Forms will be returned if patient portion is not completed.

 

•  Patient must leave physicians portion blank.  This is to be completed

    by the physician only!  Forms will be considered null and void if completed

    by the patient.
